2012 Spring Just 4 Kicks Soccer Program
Registration forms
Attached you will find registration forms for the CSC Spring soccer “Just 4 Kicks” program. Catered for pre-K through second grade. Please note there is a medical release that must be signed on th back of the form. Forms are also available to download at columbiasoccerclub.us
On the registration form, where it says “Div.,” please enter current grade, i.e., pre-K, K, 1 or 2. Only children ages 4 and up are eligible.
All forms must be mailed to CSC at PO Box 435 Columbia IL 62236 along with payment. You can also pay online at the CSC web site: columbiasoccerclub.us with a convenience fee of $5 by using PayPal. Registration dates This soccer program is designed for new or novice players to learn basic skills for playing soccer. Experienced instructors will provide a fun learning environment for your child. Each session will be one hour in length. The season will be six weeks in length and on Tuesday nights from 6-7 PM. Dates are March 20, 27, April 3, 10, 17 & 24. The deadline for the registration forms and payments to be received is March 16th. Any registration received after March 16tth will incure a $10 late fee.
